it means that when carcinoid tumor is present in the liver then the S/S are not seen cuz the 5HT that it produces undergoes metabolism in the liver. when present outside the liver like in the GI tract thats when u see the flushing etc.. all the S/S.
an first pass metabolism per se means that the concentration of the drug is greatly reduced before reaching the systemic circulation since it is undergoing metabolism in the liver

If . the primary tumor is from the GI tract (hence releasing serotonin into the hepatic portal circulation), carcinoid syndrome generally does not occur until the disease is so advanced that it overwhelms the liver's ability to metabolize the released serotonin.

So carcinoid tumor to produce symptoms has to bypass the hepatic metabolism.
