Hello everyone,
Am an IMG from India. Joined mbbs in August 2003. My internship was from march 2008 to march 2009. finished my degree on March 2009.got my degree certificate (issue date) on december 2009.
then did a postgraduation MS ophthal from May 2011 to July 2014.

Can anyone give definite answers for the following queries in my application:

Item 8 - National ID no - Should I leave it blank or give my passport no?

Item 18:
Date you completed requirements for the final medical diploma - March 2008 or 2009?
No of years attended - 6 or 5.5 or 5.6?
Graduation date - March 2008 or 2009?

Item 21 - Clinical clerkship - Do I give my postgraduate details here or leave it blnk?
