Certificate of Identity / Registration of the State for Step 3 - question !!

What am I supposed to put in the field " State of / County of " - i am currently not in the USA!? Does the CID have to be notarized in the US only??

Also, if i register my Step 3 exam in Connecticut and supposing i pass, can i still apply for residency programs across the country even if I haven't completed any prior training? Basically I am asking if i pass with Connecticut as my state listed, am I allowed to only apply to CT residency programs???
