I just subscribed for 6 months and I need to know where to write my notes... is it best to simply type my notes out or Microsoft Word or should I annotate MTB 2 for internal medicine and MTB 3 for all the rest??

It depends on what works best for you. Some prefer using other resources like MTB or Step up to Medicine and then integrating u world notes pertinent to each topic. Other prefer just making u world notes separate and organizing the information to their own liking. But if you are planning to use other books, I recommend just integrating the u world notes.
